Emolientul alb, normal si natural, ce apare sub preput se numeste smegma. Smegma este curata, nu murdara, si este necesara. Hidrateaza glandul si il mentine moale si umed. Proprietatile sale antibacteriene si antivirale ajuta la mentinerea penisului curat si sanatos. Asemanator, si femeile produc smegma, de fapt toate mamiferele.
Prin analize histologice, de laborator, s-a gasit ca smegma contine celule de piele, emolienti, lisozome si anticorpi puternici, toate acestea produse de preput.
Emolientii au rol de hidratare a glandului, pentru placere erotica maxima.
Lisozomele, prezente si in lacrimi si laptele de mama, au
rol antibacterian

protectie impotriva infectiilor,
inclusiv HIV

Cand devine in exces, smegma se indeparteaza cu apa
fara sapun, atat cat permite retractia preputului, la fel cum se
procedeaza cu secretiile oculare. Sapunul este
contraindicat.
Barbatii circumcisi nu produc smegma pentru ca preputul lipseste. Glandul nu contine glande sebacee pentru hidratare.
Cei decalotati prematur, la fel, nu produc smegma, deoarece glandele sebacee din preput au fost afectate.
Astfel, si unii si ceilati sint la risc mai mare de infectii penile.
Un rol asemanator secretiilor genitale (smegmei), de igienizare si protectie anti-virala si anti-bactieriana a penisului, il au secretiile oculare (lacrimile),
pentru ochi.
